Disaster Risk Management: Space Shuttle Disaster and Organization Risk

After the 1986 Space Shuttle Challenger Disaster, an O-ring sealing joint failure in the right solid rocket booster was given as a primary technical reason of the disaster. More importantly, management structure at the National Aeronautics and Space Administration created a situation where risk was increased. For this assignment, please name and explain two (2) ways in which the management structure at NASA contributed to the risk leading to the deadly explosion in 1986.
