Disaster Risk Management: Space Shuttle Disaster and Organization Risk

After the 1986 Space Shuttle Challenger Disaster, an O-ring sealing joint failure in the right solid rocket booster was given as a primary technical reason of the disaster. More importantly, management structure at the National Aeronautics and Space Administration created a situation where risk was increased.
